- Age Restrictions: Up to Age 65.
- Uninsured
- Income level up to 185% of Federal Poverty Level
- No residency requirement (city, county, state, or country)
- No Medicare or Medicaid clients (unless "underinsured" for services we can provide-call Director)
- Proof of income by 2nd visit - last year's tax forms preferred.
- $20 for supplies for dental extractions
- $5 per month for generic meds provided
- $10 for box of 50 test strips (to go with free glucometer provided by SRCC)
Clinic Schedule
- Tuesday Attendance Cards at 2pm; card-holders 5:30pm
- Thursday Attendance Cards at 2pm; card-holders 5:30pm
- 30 attendance cards given, first come, first served, at 2pm on far right of North Central Health Dept. building at 215 10th Street, Lewiston, ID. Must have card to be seen at 5:30pm clinic
